When I travel transpac my packing list is

- Passports/Money Folder
- Laptop + Charger
- Ipod + Cable
- Camera
- Deodorant
- Toothpaste/toothbrush (unless flying on an airline I know provides in Y)
- (Before it was stolen out of my bag at EWR) - Nintendo DS

Unless you have nothing in your carryon. A computer bag plus a pillow would already consider as 2 pieces, you are not allowed to carry a 3rd piece as carryon.

Seriously, I have only seen those young kids or teenagers would carry a pillow on board, and that is on a very rare occasion. As they usually have almost nothing to carry on board, or either their parents carry all their stuff for them.

Consider someone travel in C, where they will have lie flat beds, bringing a pillow on board is not really necessary. The neck hurts only when someone travel in Y, sitting in an upright position for long hours. You will only end up attracting unnecessary attention, possibly even at immigration where they will direct you to a special lane for more checks, as there are cases before someone smuggle something to stuff it inside something like a pillow.

I traveled transpac a couple of weeks ago for the first time. The long leg (DFW-Tokyo) was in C on AA. The shorter but still five hour leg was in Y on JAL. For flight comfort I carried on (and used):
Kindle
Compression socks (highly recommend!!)
Slippers
OTC Medicines
iPhone

I brought but did not use:
Seat organizer
Eye mask and ear plugs (provided by AA)
Seat cushion
Travel blanket (provided by AA)
Snacks (provided by AA)
Toiletries (provided by AA)

I wore yoga pants and a t-shirt with a long sleeve t-shirt over it, and was very glad I layered. The cabin varied from Arctic to sauna. They gave filling meals and had snacks and beverages in between. The pillow and comforter were completely adequate; I did not even unravel my travel blanket once on the trip. I used the iPhone White Noise app with the noise canceling headphones while I slept and it worked wonders.
