US citizens: who issued your passport?

My previous passport was issued by the Honolulu Passport Agency. My current one (applied for renewal through the US embassy in Nicosia) was issued by the US Dept of State.

I've encountered the same issue as the OP. Being that I received my current passport from the American Institute in Taiwan (de facto embassy in Taiwan), on one of my visa applications I put 'U.S. Dept of State, USA' for issuing agency and city. No issues there.
The previous passport I had was issued by the U.S. Consulate General in Hong Kong, Hong Kong (and denotes as such, since it was before the age of the biometric passport). I never had to apply for a visa using the passport but I'm sure I would have had some issues with certain countries when applying for a visa.
