USAA and my visa from fidelity brokerage usually give me the interbank rate plus about 1%
This has applied on charges of as little as a few dollars and withdrawals from foreign atms of hundreds - without any "service fees" (in fact USAA gives a 1/2 % rebate of cc charges)
