How about double upgrades using certs?
Being able to call at midnight Eastern Time to snag an upgrade ahead of everyone else.
Both cheese and desert in Business class.
Linens on the tray table domestically.
At least a snack between LAX-SFO in first.

...flying F trans-pacific on a Y consolidator ticket. Y to C on old mileage awards (15k each way--I believe), and then C to F on operational upgrade (based on old system of YTD mileage).

Pass Plus (50K miles for $15000, good for 2 years or when the miles ran out).
Pass Plus was a mileage bank good for travel with NO advanced notice on a seat availability basis. Might still exist, but the $$$ are way more.

The paper version of what is similar to a current UA HK-49 that I used to get from Sheraton each year because I was in their (pre Starwood) program as well as a Premier Executive...
Upgrading the noonish DC-10 from ORD-SFO for just $50 worth of coupons, when the lunch itself was really worth $25 of it.
Upon having the seat next to me open in the old J seats on an IAD-ZRH 767, being able to pull the divider table out from between the seats, putting it into the overhead bin with my briefcase, spreading out across both seats, and completely confusing the cabin crew.

OK. No flames please because I already know I'm in a small minority......
The absolute #1 thing I miss most is the smoking section. If there was ever one place/time where I wanted to smoke the most, it was when flying. I realize that majority rules, and I harbor no ill feelings for those who don't smoke, and/or are offended by the smell of tobacco smoke, but I am still very, very sad about this.
There are many other things that I believe have gone downhill in the past few years, but none of greater impact to me personally. Assuming that the universal no smoking policy is a "10", the next closest problem for me (new security issues) is a "5".

While the ammenities in the RCCs are more abundant, the service in the 1K rooms was far and away better. The 1K concierges took personal care of every 1K traveling from the hubs (ORD/2, DEN, SFO, IAD, LAX) and SEA. I got to know many of the 1K concierges, specifically @ DEN, ORD and SEA. I miss these people, especially during problem days.
